All computer users (including occasional users and call center staff) are welcome to fill out a confidential online RSI questionnaire:

All respondents will get feedback regarding their predisposition to RSI, suggestions regarding remedial measures and a poster on RSI Prevention and Ergonomics. All those who fill out the questionnaire can enter a prize quiz on RSI awareness. We are offering 10 copies of the most popular Stretch Break software (costing $45 each; see www.paratec.com) to encourage participation in the survey. Please note: all my existing patients should also fill out the questionnaire to become eligible for the poster and prizes.
